Here you can download the forms required in the application process for black-tailed prairie dog relocation.  Be sure to read the special conditions in their entirety, as they are extensive, and should be read and considered prior to applying for a permit.  Also, please note that these applications are for prairie dogs only, and do not apply to any other species.

Completed applications and forms should be sent to the local District Wildlife Manager. Refer to the Colorado Parks and Wildlife Offices / Phone Numbers to find the office nearest you.

Wild-to-Black-Footed Ferret Recovery Program
Use this form if you plan on delivering black-tailed prairie dogs to a facility that supports the US Fish and Wildlife Service (USFWS) national black-footed ferret recovery program.
